Undergraduate Forms
Activities pertaining to a student's education need to be documented. The forms used by Computer Science and Engineering are listed below. These forms can be downloaded and printed out. Forms are to be submitted to the College office (Library 6th Floor).
FORMS FOR ALL MAJORS
- The academic petition is for college requirements only. An academic petition for late course add, drop, or change requires the university ARC academic petition form. A link to ARC forms is found at the bottom of this page.
- - used to request an evaluation of a course for substitution purposes. This applies primarily for elective courses.
- – used to request special permission to take a course along with its prerequisite(s). This is very rarely granted.

FORMS FOR COMPUTER SCIENCE AND COMPUTER ENGINEERING majors
- used to establish a contract between the student and the supervising professor for an activity outside of a classroom course. Independent Study cannot be substituted for a required class (it can be used for an elective credit). Independent Study provides an opportunity for the student to complete a project in an area not typically covered by a college course. The student must have a contract signed by the supervising professor and approved by the Undergraduate Director or designee to register for Independent Study.
– used to establish a contract between the student and the supervising professor for a supervised research experience for elective credit. The student must have a contract signed by the supervising professor and approved by the Undergraduate Director or designee to register for Supervised Research.
– used to establish a contract between the student and the supervising professor for an industry internship for elective credit. The student must have a contract signed by the supervising professor and approved by the Undergraduate Director or designee to register for Industry Internship. No more than 3 hours of CIS 4940 in a single company. See the Industry Internship page for more information.
FORMS FOR INFORMATION TECHNOLOGY AND CYBERSECURITY majors
- used to establish a contract between the student and the supervising professor for an activity outside of a classroom course. Independent Study cannot be substituted for a required class (it can be used for an elective credit). Independent Study provides an opportunity for the student to complete a project in an area not typically covered by a college course. The student must have a contract signed by the supervising professor and approved by the Undergraduate Director or designee to register for Independent Study.

The office of the Registrar has a forms page. The Academic Regulations Committee (ARC) forms are used for readmission, late or retroactive drop, total withdrawal from entire term, late registration, late add, even exchange, and other petitions. ARC forms can be found here. For late adds, late or retroactive drops, and total withdrawals, a completed Instructor's Documentation form is required.
